Contractor Qualification Forms

Proof of insurance, in the form of a certificate of insurance with limits, are required:

• General Liability (required limit $1M)
• Automotive Liability (required limit $1M)
• Excess Liability (required limit $1M)
• Worker’s Compensation must be shown

Please mail a blanket certificate or email a copy to info@rossiconstruction.com.

Please note that your certificate of insurance must include the following:
"Rossi Construction Co., Inc. must be listed as an additional insured with respects to all work performed by or on behalf of the sub-contractor."

Workers compensation and automobile insurance must be carried by all subcontractors and noted on the certificate.

Automobile liability must be listed also before your vehicles are allowed on our jobsites.

A 30-day notice of cancellation should be indicated on the certificate.
